Appointments
Our office attempts to schedule appointments at your convenience and when time is available. Preschool and young children should be seen in the morning because they are fresher, have more energy, and are more attentive. Children in school that require extensive dental treatment should also be seen in the morning for the same reasons.

We know that all children are not alike; therefore we tailor our services to your child as an individual. Because of this, appointments may vary in frequency, length of time, and time of day. At your new patient visit we will discuss a treatment plan created specifically for your child and schedule appointments as needed for treatment. Each appointment scheduled is time reserved specially for your child. If you are unable to keep an appointment, kindly provide us with at least 24 hours notice so that this time may be offered to another patient who needs our care.


Finances
Payment for professional services are due at the time dental treatment is provided unless previous arrangements have been made. We accept cash, personal checks and most major credit cards.

If your child’s treatment requires several visits, you will be given an estimate of our fees and our staff will discuss and make definite financial arrangements prior to the appointments.

For treatment that exceeds $200 we offer financing options through Care Credit and/or BeWell by FTL Financing. All arrangements for use of the financing options must be completed before the day of treatment. If payment for the estimated cost of treatment is not paid on the date of service, we reserve the right to reschedule treatment for another day. Dental Insurance Benefits
Dental Benefits are constantly evolving as regulations continue to develop. Because of the always changing landscape of dental benefits, please familiarize yourself with your plan. We work hard to help you discover your benefits and coverage levels, however, your dental benefit company has the final determination of coverage and patient responsibility.

When discussing your benefits, dental coverage typically breaks treatment into three categories: Preventative, Basic Restoration (fillings), and Major Restoration (crowns). Most insurances require a deductible to be paid on Basic and Major Restorations. Finally, all insurances have an annual benefit maximum. This means that once they have paid their maximum, you are responsible for the remaining costs.

In an effort to assist our patients with getting the most out of their dental benefits, we will file claims for you. If we do not have accurate information on file or if your benefits expire before the day of treatment, and your dental benefit company does not pay anything, you are responsible for any unpaid balance.


Privacy
Our Healthcare Practice takes patient privacy matters seriously. We work hard to meet and exceed all existing rules and regulations and will work to keep you informed regarding our office policies and your personal rights regarding privacy.

We are required by federal and state law to maintain the privacy of your health information. We are also required to provide you with a Notice of Privacy Practices (“Notice”) which outlines our privacy practices, our duties, and your rights concerning your personal health information. We must follow the privacy practices described in this Notice while it is in effect. This Notice takes effect on November 11, 2004, and will remain in effect until we replace it, at which time we will issue a new Notice to patients indicating a new activation date. Please refer to our Forms, at any time, to view the most up to date Notice.

We use and disclose health information about you for treatment within our practice, for general healthcare operations, and payment collection. That means your information is available to our immediate staff, and to other practitioners who we may refer you to for additional treatment. This includes, but is not limited to, other healthcare specialists such as surgeons, laboratories and the like. We will exercise our judgment in only distributing the minimum necessary information needed when sending health information to any outside Associates.
